APPOINTMENTS

Schedule your visit

Choose a free introductory call, your first medical appointment, or a follow-up. Medical care is available for adults and adolescents 12 and older who are physically in California or Kentucky.

The focus can be obesity treatment, a lifestyle medicine consultation, primary care, a chronic condition, a complex case, or an independent second opinion. Standard care costs $200 for the first month; a Comprehensive Medical Review costs $500 for the first month. From month two, either option is $200 per month. We agree on the appropriate option and scope before payment.

Which appointment should you choose?

Free introductory call · 15 minutes

Start here if you would like to meet the doctor, ask how the practice works, and decide whether to begin care. If you have extensive records or several questions requiring a comprehensive review, we can discuss the scope and first-month option before you pay. Medical assessment and treatment take place during a medical appointment.

Book a free introductory call

First medical appointment · up to 60 minutes

If you are ready to begin care, book an initial consultation directly. You do not need an introductory call first. This visit includes a medical assessment, review of relevant information, and a plan for your care.

Standard care costs $200 for the first month. A Comprehensive Medical Review costs $500 for the first month, including the agreed records review, this consultation, a written assessment and plan, and continuous work on your case throughout the month. Each consultation lasts up to 60 minutes. We agree on the scope before payment; the $500 total does not carry a separate $200 charge. Care then renews automatically at $200 per month unless you cancel.

Follow-up appointment · 15–60 minutes

Choose this if you have already had your initial consultation and are continuing care. Follow-up visits are for reviewing progress or results, discussing your treatment plan, and agreeing on next steps. Choose the appointment length recommended by your doctor. Available options are 15, 30, 45, and 60 minutes.

How your appointment works

Your initial consultation takes place by secure video connection. Follow-up consultations may take place by video or by phone. For a video visit, the doctor will send you a link and instructions for joining before your appointment. For a phone follow-up, the doctor will call the number you provide at the scheduled time.

After you book your initial consultation, the doctor personally sends you the questionnaires, payment link, and preparation instructions. The first-month option, scope, and fee are agreed before payment. Your first month begins when payment is made, including preparation and records review; a later appointment does not delay its start.

Daily appointment hours:

California — 8 am–2 pm Pacific during daylight saving time, 7 am–1 pm during standard time.

Kentucky — Eastern Time areas (including Louisville and Lexington) — 11 am–5 pm Eastern during daylight saving time, 10 am–4 pm during standard time.

Kentucky — Central Time areas (including Bowling Green and Paducah) — 10 am–4 pm Central during daylight saving time, 9 am–3 pm during standard time.

Patients receiving ongoing care can send medical questions through the designated patient platform. I respond within 24 hours and usually much sooner. Temporary unavailability is covered by the vacation and illness policy. Messages are not monitored continuously and must not be used for emergencies. If care cannot wait, use an appropriate local urgent or emergency service.

Before your medical appointment

  • You are 12 or older.
  • Routine appointments for patients aged 12–17 are arranged by a parent or legally authorized representative; see the adolescent appointment process.
  • You will be physically in California or Kentucky during the consultation.
  • You understand that some concerns require local or in-person assessment.
  • You have reviewed and agreed to your first-month option and total fee — $200 for standard care or $500 for a Comprehensive Medical Review — the automatic $200 monthly renewal from month two, and the payment and cancellation terms.
  • You do not have Medicare.

Prepare for the appointment

Choose a private location with a reliable connection. Have identification, your current physical address, a callback number, your medication list with doses, and the records relevant to your main questions available.

For a complex case, prepare a short timeline and identify the decisions you would like help with. Before payment and detailed review, we agree on the questions, records, and scope of the Comprehensive Medical Review.

We agree on priorities; an extensive medical history may need more than one consultation. You will receive instructions for the clinical platform and consent process before the visit.

What you pay for

Your agreed first-month fee is paid in advance: $200 for standard care or $500 for a Comprehensive Medical Review. Both include a consultation of up to 60 minutes, a written assessment and plan, and follow-up throughout the first month. The $500 option also covers the agreed extensive initial analysis and continuous work on your case during that month: reviewing new information, reassessing the agreed questions, updating the plan, and communicating between appointments. There is no additional $200 charge for the comprehensive first month. Messaging and medically appropriate, scheduled follow-up consultations are included without a limit on their number; each consultation lasts up to 60 minutes. Clinical messages receive a response within 24 hours. Temporary unavailability is covered by the vacation and illness policy.

The first month begins when payment is made, including preparation and records review. From month two, either option renews automatically at $200 per month unless you cancel.

You can cancel at any time before the next renewal, without a cancellation fee; care continues through your paid period, subject to the applicable early termination and refund terms.

Medications, testing, imaging, and services from other providers are paid separately. Payment, appointment cancellation, subscription cancellation, and applicable refund terms are provided before payment. Eligible uninsured or self-pay patients can request a Good Faith Estimate.
