PAYMENT INFORMATION
Payment, cancellation and refunds
$200 for a standard first month or $500 for a comprehensive medical review and first month of care. From month two, either option renews automatically at $200 per month unless you cancel. Your first month starts when you pay the full first-month fee.
Your plan and when it starts
A standard first month costs $200 and includes an initial video consultation with Alexander Oparin, MD, of up to 60 minutes, a written assessment and care plan, and one month of follow-up.
The comprehensive first month costs $500. It is intended for substantial initial work, such as bringing together years of records, evaluating several interacting conditions or conflicting recommendations, or reviewing a complex treatment history. It includes review of the agreed medical records, an initial video consultation of up to 60 minutes, a written assessment and care plan, and continued work throughout the first month: reviewing new information, refining the plan, messaging and follow-up consultations.
We agree on the questions, records, scope, timing and total first-month fee before payment and detailed review. The $500 is the total for the comprehensive first month, with no additional $200 charge. Having several diagnoses does not automatically require this service; a focused second opinion that fits a standard consultation can remain within the $200 standard first month. Additional chargeable work outside the agreed scope requires your prior agreement.
Both options include messaging, review of relevant results, treatment adjustments and updates to your plan, and scheduled video or phone consultations as medically appropriate. There is no limit on the number of messages or follow-up consultations; each consultation lasts up to 60 minutes. Follow-up depends on your clinical needs. The monthly fee does not vary with the number of visits or messages you use.
Clinical messages receive a reply within 24 hours, subject to any temporary communication arrangements agreed with you in advance for a planned absence. Messages are not monitored continuously and must not be used for emergencies. For an emergency, call 911 or go to the nearest emergency department.
Medications, laboratory tests, imaging, procedures, specialist consultations and other outside services are paid separately. There is no annual plan or separate enrollment fee.
Payment and automatic renewal
You pay the first-month fee of $200 or $500 in advance. Your first month and the agreed work, including preparation, start when that fee is paid in full. A later appointment does not postpone the start of the month. From month two, your subscription renews automatically at $200 per month unless you cancel. Before payment, you receive the first-month price, the renewal amount and timing, and cancellation instructions. Automatic renewal requires your express affirmative authorization; booking an appointment alone does not authorize recurring charges.
After you book through Cal.com, I personally send you the questionnaires, the payment link, the video appointment link and instructions for your visit. Review the agreed scope, price, renewal and cancellation terms before paying.
Cancellation and refunds
You may stop automatic renewal at any time without a cancellation fee. Use the cancellation link in your payment confirmation, or email info@oparinmedical.com. To prevent the next charge, submit your cancellation before the next renewal. Email cancellations take effect when received, regardless of when we reply.
Stopping renewal leaves the agreed care available through the end of your current paid month. There will be no further monthly renewals. Canceling or rescheduling an appointment does not, by itself, stop your subscription.
The monthly fee covers the agreed availability of care as well as the consultations and messages you use. Except where applicable law requires otherwise, we do not provide full or prorated refunds solely because you stop renewal, use fewer services, or choose not to use care that remains available as agreed.
You or your legal guardian may also give written notice by email to terminate the agreement before the paid period ends. There is no termination fee. Upon termination, we will return all unearned prepaid fees that applicable law requires us to refund to you or the appropriate payor.
For the comprehensive first month, the scope and itemization used to account for the initial review and follow-up are agreed before payment, within the fixed $500 total. Paying starts the month; it does not, by itself, mean the whole $500 has been earned. If a refund is required, the calculation accounts for the agreed work actually completed and unearned prepaid care, without counting the same work twice.
Refunds are otherwise provided only where required by applicable law, including any required refund for incorrect or unauthorized charges or care the practice failed to provide or make available as agreed. Nothing in this policy limits any cancellation, refund, billing-dispute or other patient right under applicable law.
This policy applies to patients in California and Kentucky.
Appointments and connection problems
Canceling or missing an appointment, or a connection problem, does not automatically restart or extend the paid month. Contact me to discuss the next step if an appointment needs to be moved or the connection fails. This does not remove your rights if the practice cannot provide the agreed care.
Vacation or illness
I give advance notice of planned vacations. We address ongoing treatment needs ahead of time and agree on how you can obtain care if you need it before I return.
If illness or another temporary absence prevents me from providing the care agreed under your membership and appropriate coverage has not been arranged, I will notify you and discuss the next steps. Refunds are provided only where required by applicable law, as described in the “Cancellation and refunds” section.
Insurance, estimates and eligibility
This subscription is not health insurance or a health benefit plan and does not satisfy any individual health coverage mandate that may apply. Insurance reimbursement of physician fees is not guaranteed. Medications and outside services have their own prices and coverage arrangements.
Care is for adults and adolescents aged 12 and older who are physically in California or Kentucky when receiving it. The practice does not currently accept patients with Medicare, including on a self-pay basis.
Eligible uninsured or self-pay patients have the right to a written Good Faith Estimate. Website prices do not replace an individual estimate. See Good Faith Estimate information and telehealth information.
